- Who governs asbestos training course approval in Ohio?
The Ohio Department of Health under chapter 3710 of the Ohio Revised Code and chapter 3701-34 of the Ohio Administrative Code.
- Who must apply for asbestos training course approval?
Any person who will conduct or offer to conduct any initial or refresher asbestos training course in Ohio.
- How do I apply?
To receive training course approval you need to complete the Ohio Department of Health Training Program Application. Please read the application carefully. Copies of the application are also available by contacting the Ohio Department of Health Asbestos Program at (614) 466-0061 or e-mail us at ASBESTOS@odh.ohio.gov
- What do I need to include in my application?
For training course approval, there are 10 basic elements, which must be submitted in order to process your application. Refer to chapter 3701-34 of the Ohio Administrative Code.
- Completed application with original notarized signature and (photocopies or facsimiles will not be accepted).
- Documentation that the training course currently has full or contingent approval by the USEPA or by a state that has an accreditation program approved by the USEPA.
- Copy of your curriculum.
- Copy of your course materials.
- Information pertaining to the number of course hours.
- Copy of your written examination.
- Certificate of completion.
- Complete list of your course trainers and their qualifications.
- Letter that clearly indicates how the training course meets or exceeds the applicable requirements set forth by the USEPA Model Accreditation Plan (40 CFR Part 763 Subpart E, appendix C) and the Ohio Administrative Code 3701-34-07
- Check or money order for the fee listed on the application
- $900.00 for an initial training course
- $300.00 for a refresher training course
- When does my training course approval expire?
Training course approvals expire one year from the date of approval.
- How does my training course obtain final approval?
Once the director approves a training course application that course is granted contingent approval. The director shall grant final approval of a course only after an on-site audit by the Ohio Department of Health reveals that the course substantially satisfies the criteria in chapter 3701-34 of the Ohio Administrative Code.
- Does 2-Hour Awareness training or Operations & Maintenance training have to be approved through the Ohio Department of Health?
No. The Ohio Department of Health does not approve the 2-Hour Awareness training or the Operations & Maintenance training. However, both training courses do have to comply with Federal EPA and OSHA Standards.
