How to Get a Certified Copy of an Ohio Birth or Death Record

Internet Orders

The easiest way to obtain a certified copy of an Ohio birth record, death record, or acknowledgment of paternity affidavit is to place your order online with the Bureau of Vital Statistics.  Our online ordering portal will ask for enough information to identify your requested record and accepts credit/debit card payments.  Please allow processing time of up to two weeks to receive your certificates and be aware that our online service does not allow for rush orders or cancellation of submitted orders. Click here to go to our online order portal.  If you are requesting a birth certificate for dual citizenship or international use we recommend placing your order in person or through the mail. 

In-Person Orders

If you live in the State of Ohio ordering your certificates in person may be a fast and convenient option.  You can request a certified copy of an Ohio birth record from any of our 110 vital statistics offices.  Certified copies of death records may be obtained from the local Vital Statistics office in the city/county health district where the death occurred or from the state Vital Statistics office.  Hours of operation and local fees may vary across the state; for a statewide directory of local VS offices click here. 

Same-day certificate service through our state Vital Statistics office in downtown Columbus is available 8:00 AM to 5:00 PM Monday through Friday.  We have application forms available in our lobby and accept payment in cash, credit card, check or money order.  Birth and death certificates are usually issued within twenty minutes.  For full instructions on ordering at our office click here. 

Mail Orders

Our office does accept orders through the mail for birth and death records, acknowledgment of paternity affidavits, and other requests.  Please allow processing time of at least four (4) weeks for mailed requests.  Payment for certified copies of our records should be submitted by check or money order only; please do not send cash.  Click here to review complete mailing instructions.

How do I request a copy of my marriage/divorce record?

How do I request a copy of my adoption file? 

Requests for copies of sealed adoption records can only be submitted in person at our state VS office or through the mail.  As the laws governing the release of this information vary according to the year the adoption was finalized and the relationship of the requester to the adopted individual, we recommend clicking here to review adoption information and access the appropriate forms before submitting a request.
