As part of the Statewide initiative to move to Microsoft Office 365, the Ohio Department of Health, Office of Management Information Systems, is in the initial phases of planning and preparing for this transition. One of the first items in this project is the transfer of the current ODH Email to the 365 Cloud. At this time we do not have an exact date of the email migration, but wanted to provide you with notice so you can be on the lookout for future communications regarding this project. A number of details that may impact you and how you access your e-mail will be forthcoming. We are looking forward to working with you and the opportunities that this new platform will provide.
Health Insurance Portability and Accountability Act (HIPAA)
HIPAA Privacy Rule requires each covered entity or hybrid entity to report any breach of unsecured protected health information to the Office for Civil Rights (OCR) at the U.S. Department of Health and Human Services (HHS). 45 CFR 164.408. Breaches affecting more than 500 persons must be reported immediately otherwise, breaches must be reported annually within sixty (60) days of the end of the calendar year. Accordingly, the reporting deadline for breaches occurring in 2013 is March 1, 2014. The forms can be found here:
Affirmative Action Guidance
Ohio Revised Code DOES NOT require an entity to submit an affirmative action plan when:
A. The total contract amount is $0.00.
B. The entity is acting in a government capacity on the behalf of or as an extension of the Ohio Department of Health. This determination will be made by ODH on a contract by contract basis. In most cases, an affirmative action plan will be required. Local health districts are acting as an extension of ODH any time they are a subgrantee for a grant.
Ohio Revised Code DOES require an entity to submit an affirmative action plan to the Ohio Department of Health- Contract Unit when:
A. The contract amount is $.01 or above.

An affirmative action plan needs to registered and updated annually with the Equal Opportunity Division of the Department of Administrative Services when a contract is $2,500.00 or above.
