HIV Home Health Program
Care and compassion when you need it most.
Ohio’s Ryan White Home Health program offers skilled nursing care when needed in the home environment and/or homemaker services to assist with activities of daily living.
With the assistance of homemakers, home health aides and/or skilled nursing visits, we may be able to help people with HIV/AIDS stay at home or keep them from having to go to the hospital.
The Home Health Program, like all other HIV Care Services programs, helps to provide continuity of care and improve quality of life.
To access the program, a Ryan White case manager must:
Identify the need for services for an eligible client
Determine if the client is receiving Medicaid or is eligible for Medicaid
If possible, assist the client to access services through the Ohio Department of Job and Family Services first to ensure the Ryan White program is the payor of last resort
Initiate a home care referral online and notify the HCS Home Health program coordinator
The Home Health program coordinator will contact a local Home Health Agency (HHA) and arrange for an assessment of the client. If the client is eligible and qualified for home care services, the HHA staff and HCS coordinator work together to establish appropriate services in the home.
Consider the program if your client needs:

Skilled nursing care in the home

Assistance with activities of daily living (ADL)

Assistance with ADL in addition to home nursing provided by other programs

Coverage for gaps in services while Medicaid is pending

Post-op recovery care

Respite care
