Access to Ohio Cancer Incidence Data
Ohio cancer incidence data, i.e., information on cases of cancer newly diagnosed among Ohioans during a specified time period, from the Ohio Cancer Incidence Surveillance System (OCISS) at the Ohio Department of Health (ODH), are available to researchers and the public in multiple formats: tabular; published cancer statistics; non-confidential customized analyses and datasets; and confidential customized analyses and datasets. Access to Ohio cancer incidence data, by format, can be obtained through the following procedures:
- Tabular Cancer Incidence Data - are available through ODH's Information Warehouse
(Information Warehouse).
- Published Cancer Statistics - are available through OCISS reports (Reports) and
cancer fact sheets (Fact Sheets).
- Non-confidential Customized Analyses and Datasets - may be requested by:

The following reference document and form are available to make application for non-confidential customized analyses and datasets: ODH/OCISS Data Items Available for Surveillance and Research (Data Items), a complete listing of available cancer data items and information about how to access the North American Association of Central Cancer Registries (NAACCR) Data Dictionary and ODH/OCISS Data User's Agreement (Data User Agreement).
- Confidential Customized Analyses and Datasets - are available for research to further the prevention and control of cancer. To obtain confidential data, approval must be obtained for the OCISS and the ODH Institutional Review Board (IRB). Confidential cancer data may include individual-level data and/or tabular data that involve highly specific population categories and/or small cell sizes. Approval may be obtained through the following procedures:

ODH/OCISS Data User's Agreement (Data User Agreement). ODH/IRB Application for Protocol Review (IRB Form).
- Review the Data Items list and complete the IRB Form and the
Data User Agreement.
- Attach a copy of the formal scientific protocol for which data will be used as approved by your IRB.
- On the IRB Form or in your prtocol, list each researcher who will have access to the data for this study and attach a curriculum vita and a signed
Data User Agreement for each researcher.
- On the IRB Form indicate the following:
Details on how the data will be protected (locked in drawers, located on PCs and password protected, electronically transferred in encrypted format only, not faxed, etc.). Length of the project period. How data will be disposed of (on paper, diskettes/CDs, PC hard drives, mainframes, etc.).
- Specify on the IRB Form the diagnosis year(s) (select from 1996 - most recent) for which you want data.
- If requesting individual-level case data, please select from the Data Items list which data items you want to receive. List these somewhere on the
IRB Form, in the protocol or as an attachment.
- Attach other IRB approvals received to date (from your institution, state cancer registry, health department, federal agency, etc.). These should be received prior to applying to ODH's IRB.
- Mail the documents to the address given on the bottom of the IRB Form and a copy of the documents to:
