Hearing Aid Dealers and Fitting Licensing Board - Duplicate Licenses and Permits

Each individual or corporation who holds a hearing aid dealers and fitters license or trainee permit shall display the license or permit in a conspicuous place in his place of business at all times. If the individual or corporation maintains more than one office a duplicate license or permit shall be posted in each location. To obtain a duplicate license or permit, an application, or written request must be made to the Hearing Aid Dealers and Fitters Licensing Board, 246 North High Street, Columbus, Ohio 43215. The written request must include the address of the location where the duplicate is to be placed, or stated that the duplicate is to replace a lost license or permit. The application or written request must be accompanied by a check or money order, payable to Treasurer, State of Ohio, in the amount of $16.00, for each duplicate license or permit requested.

If the holder of a trainee permit changes supervisors, that person shall submit an application to the Board for a duplicate permit. The application must be signed by the new supervisor, and must be accompanied by a check or money order, payable to Treasurer, State of Ohio in the amount of $16.00.
