
Hearing Aid Dealers and Fitting Licensing Board - Trainee PermitsThe trainee permit is intended to allow individuals the opportunity to fit and sell hearing aids while working under the supervision of a person who is licensed as a hearing aid dealer and fitter. The trainee permit is issued for one year, and can be renewed one time. At any time during the trainee period the holder of the permit can apply for his license and the examination. A trainee can become licensed upon the successful completion of examination and the other licensing requirements. How to Apply for a Trainee Permit: An individual who wants to obtain a trainee permit must meet the following requirements:
To become a trainee the candidate must submit a properly completed application to the Hearing Aid Dealers and Fitters Licensing Board, 246 North High Street, Columbus, Ohio 43215. The application must include a statement from a physician confirming that the candidate is free of contagious or infectious diseases, and a check or money order, payable to the Treasurer, State of Ohio, in the amount of $150.00. If a trainee changes supervisors, that person must submit an application for a duplicate permit to the Ohio Hearing Aid Dealers and Fitters Licensing Board, 246 North High Street, Columbus, Ohio 43215. The application must be signed by the new supervisor, and accompanied by a check or money order, payable to Treasurer, State of Ohio, in the amount of $16.00. Last Updated: 1/6/08 |
