online.  This information will be available only for a limited time: 
September through Dec. 31, 2009. 

Schools

ODH requests all schools with Internet access submit their
immunization reports online. ODH is in the process of phasing out the
traditional hard copy reports. You may report the kindergarten or 1-12
status online by accessing the information below. In order to do this,
you must enter the school IRN number to access the online reporting
form for your school.  If this does not work for your school, or you have
a new school, you will need to complete and submit the forms by mail
as done in previous years.  Please call 1-800-282-0546 for the forms.

Child Care/Head Start

You will be able to report the Child Care or Head Start status by accessing
the information below.  In order to do this you must use the six-digit
licensing number to access the report for your center or program.  If this
does not work for your center or you have a new center or program,
you will need to complete and submit the forms by mail as done in
previous years.  Please call 1-800-282-0546 for the forms.
