| X-ray Equipment Registration
To register your radiation-generating equipment with the Ohio Department of Health, you must submit the following:
- a completed Application for Registration of Radiation-Generating Equipment at least 30 days prior to handling the equipment, and
- a non-refundable check in the amount of $262.00, made payable to the Treasurer, State of Ohio.

Amendments/Changes:
To change any information contained on the "Application for Registration of Radiation-Generating Equipment" form (e.g. number of x-ray tubes, site location, etc.), the Director must be notified in writing 15 days prior to the change occurring. A completed "Amendment of Radiation-Generating Equipment" form will serve as notification to the Director.
