Community Health Care Facilities & Services
Public Services Available:

  • Staff investigation of complaints pertaining to any of the entities regulated through the Bureau of Community Health Care Facilities and Services may be requested by contacting a toll-free phone hot line at the Ohio Department of Health’s centralized complaint intake unit. If you are registering a complaint about a provider or supplier who is certified in the Medicare Program, call 1-800-669-3534. If the complaint pertains to a state licensure program participant as shown in the list immediately above, call 1-800-342-0553. You may give your name (if you desire a response to your allegations) or remain anonymous.
     
  • Bureau staff members are available as speakers at no charge to established organizations with an audience of 50 or more participants. Any topic related to the regulatory or oversight activities of the bureau may be address (e.g., the laws, rules, regulations, standards under which providers/suppliers must operate; survey and inspection processes and protocols followed in the bureau; or statistical reviews of regulatory compliance). Presentations should be limited to two hours or less. To request a speaker, please contact the Division of Quality Assurance Speakers Bureau at 1-614-752-9908.
     
  • While survey or inspection reports pertaining to the providers and suppliers regulated through this bureau are generally public documents, the bureau does not provide it to the public directly. Requests for a copy of a survey or inspection report may be directed to 1-614-644-7238 or in writing to Bureau of Information and Operational Support, Attention: Records, 246 North High Street, 3rd Floor, Columbus, Ohio 43215. There may be a minimal cost for reproduction of records and requests may take 4 to 6 weeks for delivery.
