Hearing Aid Dealers and Fitters Licensing Board


Ohio R.C. § 4747.02 : Unlicensed practice

“No person, firm, partnership, association, or corporation shall, on or after July 1, 1970, engage in the sale, practice of dealing in or fitting of hearing aids, advertise or assume such practice, or engage in training to become a licensed hearing aid dealer or fitter without first being licensed as provided in this chapter.”

Ohio A.C. § 4747-1-08 : Application for license and permits

“(A) Every person requesting an application for a license or trainee permit shall be furnished the necessary forms, a copy of the Revised Code pertaining to licensing of hearing aid dealers and fitters, and such other information or questionnaires as the board may deem desirable.”

Applying for a Business License

Before applying for a Hearing Aid Dealers and Fitters business license, the firm must be registered with the office of the Ohio Secretary of State (SOS). The firm’s SOS registration number is required before the business license application can be processed. Any ‘doing business as’ name (DBA) requires its own SOS registration number.

In addition, a business license application cannot be processed without including the firm’s Federal Tax Identification Number, or EIN, as well. Sole proprietors may use their social security number. Contact the IRS to get an EIN if you don’t already have one.

Getting Your Trainee Permit

From Ohio A.C. § 4747-1-08(E): “Each application shall be signed by the applicant and sworn to before a notary public. In the case of a trainee permit, the supervisor’s name, address, license number, and signature shall appear. The supervisor’s signature must be signed and sworn to before a notary public. Every trainee shall be assigned a supervisor. The supervisor’s name shall appear on the trainee permit and card and may not be changed without the trainee first requesting in writing a revised duplicate permit from the board, which request shall include the name of the new supervisor and the effective date of the change.”

The form needs to be completely filled out and you and your supervisor need to sign the completed form in front of a notary public.

Applying for an Examination

Ohio R.C. § 4747.08 : Qualifying examination

Ohio A.C. § 4747-1-09 : Examination

From Ohio R.C. § 4747.08: “After July 1, 1970, no person shall be issued a hearing aid dealer’s or fitter’s license unless such person has successfully taken and passed a qualifying examination.”

From Ohio A.C. § 4747-1-09(C): “Responsibility for filing an application for examination is the responsibility of the applicant.”

From Ohio A.C. § 4747-1-09(F): “A person who has failed to pass all sections of the written or the performance examination within three attempts, shall be required to submit a new application for the examination and retake the examination in its entirety.”

Applying for an Individual License

Ohio R.C. § 4747.05 : Application for license

From Ohio R.C. § 4747.08(A)(1) through (A)(4): “The hearing aid dealers and fitters licensing board shall issue to each applicant, within sixty days of receipt of a properly completed application and payment of two hundred sixty-two dollars, a hearing aid dealer’s or fitter’s license if the applicant: (1) is at least eighteen years of age; (2) has not committed a disqualifying offense or a crime of moral turpitude, as those terms are defined in section 4776.10 of the Revised Code; (3) is free of contagious or infectious disease; [and] (4) has successfully passed a qualifying examination specified and administered by the board.”
