Radiologic Licensure - General Information & Fees

Radiologic License Requirements

Radiologic licenses are issued for the following categories: Radiographer, Nuclear Medicine Technologist, Radiation Therapist and General X-ray Machine Operator (GXMO). Please click on each link below for specific requirements for each category.

Radiologic License Fees

  • Initial application fee: $65
  • Renewal application fee: $45
  • Current GXMO licensee converting to a Radiographer license: $15
    Note: The Radiographer license will have the same expiration date as the current GXMO license.

All fees are non-refundable and non-transferable; acknowledgement is affirmed when signing the "Statement of Declaration" section on the application.

Summary of the Radiologic License Rule Amendments

  • The continuing education requirement has increased from six hours to 12 hours.
  • The Ohio Department of Health (ODH) no longer issues pocket license cards.  Licensees must print their license certificate from the X-ray Licensure System (XLS): Check Application/License Status/Print License.
  • Employers may verify a licensee's status forrm the XLS: License Verification.
  • ODH no longer accepts the GXMO competency form as proof of completing clinical practice assignments. All new GXMO applicants must complete at least one clinical educational course, specific to the type(s) of procedures they perform.  The completion of the clinical module(s) allows the GXMO to perform standard, diagnostic procedures using digital equipment.  GXMO applicants must submit their clinical course certificate, or if a student enrolled in a radiologic technology program of study a GXMO Clinical Module Affidavit form, with the initial application.
  • GXMO applicants must submit proof of their GXMO didactic education directly to the Cleveland Clinic in order to take the GXMO examination.  ODH no longer supplies examination registration forms to GXMO applicants.  (Contact your GXMO didactic educational course provider for more information.)
  • An applicant may take the state examination up to three times within one year of successful completion of a didactic educational program.  If the individual does not pass the state exam within the one-year period (or fails three times), the individual must retake the didactic education prior to retesting.
