Status of Application/License

In order to check the status of your radiologic license application or print your license certificate, you will need to create an account in the ODH Gateway.  The ODH Gateway provides added security to your personal information.

New ODH Gateway Users
If you have not created an account in the ODH Gateway, please click on the XLS Entry Form link below.  You will need to have an e-mail address in order to receive your User ID and temporary password.  You will be required to change yoru temporary password the first time you access the ODH Gateway.  Additionally, you will be required to update the Security Question and Answer by selecting the Personal Info tab once in the ODH Gateway.

License Verification

Please click on the link below to verify if an individual holds a valid Ohio radiologic licensure. You will need the individual's name.
