Community Heart Health Program

The Ohio Department of Health’s Cardiovascular Health Program consists of two major components: (1) Community Heart Health Projects funded by the Preventive Health & Health Services Block Grant (PHHSBG) and (2) Core Capacity CVH Cooperative Agreement from CDC.

 

The Community Heart Health Program (CHH) provides fiscal and technical assistance to 17 local projects covering 24 counties in 2006. These projects use a population-based approach to expand, and enhance heart healthy communities.

 

Using a team approach, the local project staff uses four settings for their interventions: school, worksite, community and healthcare. Their interventions encompass six modifiable CVH risk factors: nutrition, physical activity, blood pressure, blood cholesterol, tobacco and diabetes.
