Heirloom Certificates

An heirloom certificate is a legal document which can be passed from generation to generation and illustrates eight U.S. presidents from Ohio, commemorates the first flight of the Wright brothers, Neil Armstrong’s walk on the moon and the first state Capitol and office buildings in Columbus. The certificate comes in two sizes (8 ½ X 11 or 11 X 14)

To order an heirloom certificate, complete an application and include the following:

  • Name at birth

  • Date of birth

  • Place of Birth in Ohio (city & county)

  • Full Maiden Name of Mother

  • Full Name of Father

  • Size of the certificate 8 ½ X 11 or 11 X 14

  • $25.00 per certificate

    Effective October 16, 2009, the price of an Heirloom Certificate will increase to $30.00
