Adoptions Finalized Prior to Jan. 1, 1964:

Adoption Records are Open to Adopted Person, Adoptive Parents and/or Lineal Descendants with Proper Identification

Affidavit of Adopted Person: Form HEA#3011

May receive copies of the contents of their adoption file.  The file may include the final decree of adoption and/or the original birth certificate. It will not include medical history.

• Adoption must have been finalized before 1/1/1964

• Notarized affidavit is required

• Two pieces of identification (if name change or marriage, need to show linkage)

• $20 fee
