You can also obtain the birth/death record from the city or county health department office where the event occurred, provided there is no court or legal action.

For all birth records requiring no amendments (adoptions, paternity establishment, name changes, etc.), and for all death records, please allow delivery time of 3 to 6 weeks from the date the request is received in this office. Any current legal actions or corrections being done, will require between 3 to 6 months for processing.

A check or money order of $16.50 (in U.S. funds) for each certificate (birth and death only), payable to the "Treasurer, State of Ohio", should accompany each request which covers the cost for the certificate and search. (Please note that we do not accept counter checks, that is, checks that do not have the user's preprinted name and address.)
