Mail Your Request

You can obtain the birth/death record from the city or county health department where the event occurred. Some offices can take your order over the phone with a credit card and many can accept orders online through their website. Turnaround time at these smaller offices is often faster than the state office.  All city and county vital statistics offices in Ohio have access to the state database of birth records.  Click here to review our statewide directory.

When your application and fee are received by the Vital Statistics, an abstract of the birth certificate data will be printed and certified. If you need a certified book copy of your birth certificate (for foreign authentication, or if your certificate was filed more than one year after birth, etc.) please specify on your application. 

For requests of a birth record which does not require an amendment (adoptions, paternity establishment, name changes, etc.), and for all death record requests, please allow delivery time of up to three weeks from the date the request is received in this office. Amendments will take one to three months to process any corrections or legal actions.

The required fee for each certified record is $21.50; we accept checks and money orders (in U.S. funds).
