You can also obtain the birth/death record from the city or county health department office where the event occurred, provided there is no court or legal action (name change, correction, or adoption).
Locate the local health district office
The Ohio Department of Health has a contract with VitalChek Network to help people get a certified copy of a birth or death certificates using a credit card. The service provider will charge you an additional processing fee in order for you to use a credit card, as well as an additional fee if you want your document sent Federal Express/UPS. Please review the information below for how to order your certificate using these vendors.
Please note that regardless of what mailing option is requested, all certified copies will take 2-4 weeks for processing, not including mailing. For faster service, customers are encouraged to contact the local statistics office of the county where the birth/death event occurred.

VitalChek - The cost of a certified birth or death certificate from our office is $21.50. VitalChek will charge you an additional $9.95 service fee for processing your certificate request using a credit card and standard USPS mail. If you would prefer to have your certificate sent via Federal Express/UPS, the additional fee is $11.25.* Requests can be initiated online at Vitalchek.com by phone at 1 (877) 828-3101 press option 2 or by fax to 1 (877) 553-2439. A credit card or debit card is required to complete your transaction.
In addition to a credit-card number and expiration date, VitalChek requires the following information:

Mother's full and maiden name, if requesting a Birth Certificate
